About the Role:

As a Care Coordinator, you'll play a vital role in managing and coordinating care for identified  high-risk members. These services range from comprehensive care management and health promotion to transitional care, family support, and connecting members to essential community and social services. You'll collaborate with clients, families, and healthcare providers to ensure personalized, effective care that leads to better health outcomes.

Key Responsibilities:

  • Develop and Manage Care Plans: Collaborate with clients and their families to create and regularly update personalized IBHH treatment plans.
  • Resource Coordination: Link clients to appropriate services and community resources that support their health goals.
  • Advocacy: Advocate for clients to ensure they receive the necessary resources to implement their care plans effectively.
  • Monitor Progress: Oversee the delivery of services, ensuring each client’s progress is tracked and reported.
  • Document Services: Maintain thorough documentation of at least two core services per month, ensuring compliance with clinical standards.
  • Coordinate Aftercare: Arrange for post-discharge services following ER or inpatient admissions to ensure continued care.
  • Liaison Work: Serve as a point of contact between clients, families, healthcare providers, and social service agencies.

What You Bring:

  • Strong knowledge of DSM-5 and proficiency in utilizing community resources.
  • Experience working with culturally diverse groups and individuals experiencing psychiatric, substance use, or medical challenges.
  • Excellent time management and communication skills—able to handle situations with professionalism, even in times of stress.
  • Commitment to Chrysalis Health’s mission and vision, providing compassionate care and supporting the needs of others.
  • Ability to work collaboratively in a team environment while maintaining professionalism and respect.

Requirements/Education:

  • Graduation from an accredited college or university with a minimum of a bachelor's degree in psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling, or related field with at least 1-year experience working with the specific population they are applying to work with (i.e., children with serious emotional disturbances or adults with serious mental illness)
  • Valid Florida driver’s license and proof of insurance with required liability limits.
  • Must pass annual background checks and maintain a safe driving record with no more than 6 points on the Motor Vehicle Report (MV
