Position Overview: 

As a Shelter Case Manager, you will be at the forefront of coordinating admissions, conducting assessments, and developing service plans for our clients. Your responsibilities will include maintaining contact with relevant parties, ensuring compliance with medical and dental requirements, and facilitating discharge planning. You'll work closely with a multidisciplinary team to provide holistic support to those in our care. Hourly range starting at $18.00 an hour. Schedule: Monday-Friday 9am-5pm or 10am-6pm.

Key Responsibilities: 

  • Provide comprehensive case management services in alignment with organizational policies and procedures. 
  • Conduct ongoing chart audits to ensure quality assurance and compliance. 
  • Utilize community resources effectively for supportive services and discharge planning. 
  • Serve as a liaison between clients, families, referral sources, and other social service agencies. 
  • Offer in-service training to staff to enhance service delivery. 
  • Demonstrate proficiency in crisis intervention and reporting. 
  • Complete documentation accurately and within specified time frames. 
  • Collaborate with diverse groups and individuals in a professional and compassionate manner. 

Qualifications: 

 

  • Associate’s degree with a minimum of 5 years’ experience working with children OR Bachelor’s Degree preferred in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling or related field with at least two-years’ experience working with children. Preferred but not required.
  • Strong understanding of DSM-IV and DSM-5. 
  • Ability to recognize and intervene in crisis situations. 
  • Experience in providing mental health services. 
  • Excellent communication and interpersonal skills. 
  • Commitment to professionalism and ethical conduct.
